Granulomas in the liver are most commonly caused by

Tuberculosis and Histoplasmosis

Granulomatous inflammation in the liver happens when a persistent pathogen or antigen triggers a cellular immune response that walls off the offender into organized granulomas. Macrophages become epithelioid cells and often fuse into Langhans-type giant cells, surrounded by lymphocytes. Among infectious causes, mycobacteria such as tuberculosis and certain fungi like Histoplasma are classic culprits because these organisms persist and provoke this containment pattern. In contrast, viral hepatitis mainly causes hepatocellular necroinflammatory injury with lymphocytes rather than well-formed granulomas; autoimmune hepatitis typically shows interface hepatitis with plasma cells; candidiasis tends to produce neutrophilic microabscesses rather than true granulomas. Thus TB and Histoplasma account for the granulomatous liver pattern most commonly.
